We can answer:

The tattoo is NOT FAKE!

The man is Josh Marshall. To the right is his son Gabriel Marshall, who was diagnosed with a brain tumor in March 2015. This tumor was surgically removed, but this scar remained.

to ABC News[ 1 ] by saying that he wanted to get the scar as a tattoo so that the boy would not be alone when people stared at him. A tattoo as an act of solidarity and fatherly love:

In August 2015, Marshall decided to get a tattoo mimicking his son's scar. “I said, 'You know what, I'll get your scar tattooed. That way, if people want to stare at you, then they can stare at both of us.'”
